What is a Group MSA?

A Group Medical Savings Account (Group MSA) is a type of Medicare Advantage plan — but with a powerful twist. Instead of paying a monthly premium, Medicare deposits money directly into a personal savings account that the employee owns and controls. To understand what makes a Group MSA special, it helps to see how it compares to other Medicare Advantage plan types.

HMO — Health Maintenance Organization

Requires the use of a network of doctors and usually needs a referral to see a specialist. Generally lower out-of-pocket costs when staying in-network.

PPO — Preferred Provider Organization

Offers flexibility to see any doctor, in or out of network, without a referral — though staying in-network typically costs less.

PFFS — Private Fee-for-Service

Allows you to see any Medicare-approved provider who accepts the plan's payment terms. Providers may choose whether to accept the plan for each visit.

SNP — Special Needs Plan

Designed for people with specific diseases, conditions, or limited incomes. Provides tailored benefits and drug coverage for those who qualify.

Fenyx Health Standard Plans — For Employers and Their Medicare-Eligible Workforce

Available to groups of any size, in all 50 states plus D.C. $0 cost to the employer. $0 monthly premium for the enrollee. The employer's role is simply to make their Medicare-eligible employees and retirees aware of the option — the plan itself is between the enrollee and Medicare. We currently offer two Standard plans, Plan 200 and Plan 300, with most enrollees choosing Plan 300 for its larger annual deposit.

Fenyx Health Self-Employed Plans — For Business Owners & Sole Proprietors

Business owners can enroll in a Self-Employed plan for themselves while also offering the Standard 200 or 300 plans to their employees and retirees. Self-Employed plans are funded by a monthly group fee paid by the business, in exchange for a larger annual Medicare deposit into the owner's MSA account.

💼 Tax Note: Group fees paid by self-employed plan sponsors may be deductible as a business expense, while the Medicare deposit goes into your MSA account to be used for qualified healthcare expenses. Interested participants enroll through MyGroupMSA. Your MyGroupMSA agent guides every enrollee through the plan enrollment process — making it simple and stress-free from start to finish.

Funds Are Deposited

Once enrolled, Medicare deposits the annual MSA funds at the start of each calendar year — or prorated from the enrollment month for partial-year enrollments. Participants have full coverage and cash in their account to put toward healthcare expenses.

What About ERISA? — A Note for Business Owners & HR Professionals
One of the most common questions we hear — and one of the most important to answer carefully.

Many business owners and HR professionals hear "Group MSA" and immediately wonder about ERISA compliance obligations. It is a fair question. For our Standard Plans — offered to a Medicare-eligible workforce at $0 cost to the business — the arrangement is generally structured to fall under the ERISA Voluntary Plan Safe Harbor at 29 CFR 2510.3-1(j), which requires four conditions:

No Employer Contributions
Medicare funds the deposits. The business pays nothing toward the Standard Plans — the employer contributes nothing financially to the enrollment.
100% Voluntary Enrollment
The enrollee chooses to participate. No mandate, no pressure, no requirement. Enrollment is entirely the individual's decision.
No Employer Endorsement or Management
The employer's role is simply to allow MyGroupMSA to introduce the Medicare option to their eligible workforce. MyGroupMSA handles all education, enrollment, and ongoing support. The plan itself is between the enrollee and Medicare — the employer does not sponsor, administer, or manage it.
No Financial Benefit to the Employer
The employer receives no compensation, rebates, or financial consideration of any kind for making this option available.

The bottom line for HR & business owners: For employers whose offering meets the safe harbor criteria, the compliance footprint is typically much lighter than with traditional employer-sponsored coverage — often without the wrap documents, Form 5500, and SPD obligations that come with traditional group health plans. We will help you understand the structure and recommend you confirm the specifics with your benefits counsel.

📋 A Note on Self-Employed Plans
For Self-Employed Plans — where the business owner enrolls themselves and the business pays a monthly group fee — the ERISA analysis is different and depends on your specific business structure. In most sole-proprietor situations this is not an ERISA concern because the owner is the enrollee, but we recommend you confirm the details with your tax advisor or benefits counsel. We are happy to walk through this with you.

Who Qualifies?

Group MSA plans are available to a wider circle of people than most business owners realize — but Medicare's eligibility rules are specific. Here is who may qualify based on the Fenyx Health 2026 Summary of Benefits, and who likely will not.

Active Employees Age 65+

Active employees enrolled in Medicare Parts A and B and who do not have other coverage that would pay toward the MSA deductible may be eligible. Because most active employees are still enrolled in their employer's group health plan — which would disqualify them — this is a narrower group than people often expect. We will help determine who qualifies in your specific situation.

Retirees & Their Medicare-Eligible Spouses

Medicare-eligible retirees of your business who do not have other disqualifying coverage may be eligible. Their Medicare-eligible spouses may also be able to enroll in their own individual Group MSA plan, with their own annual Medicare deposit. Each enrollee has their own plan and their own MSA account — funds are not shared between spouses.

Former Employees & Retirees

Former employees who are now Medicare-eligible may qualify as an "eligible beneficiary of the group" — even if they only worked for the company briefly or left years ago. Per the Fenyx Health 2026 Summary of Benefits, eligibility is based on Medicare enrollment and coverage status, not length of employment. We will confirm eligibility with each individual before any enrollment takes place.

Businesses of All Sizes — Including Sole Proprietors

From sole-proprietor and family-owned businesses to mid-sized companies, any business with Medicare-eligible owners, employees, or retirees may be a fit. Available in all 50 states plus D.C. Government employers, unions, and employment-related associations may also qualify.

Important: Who Does Not Qualify
Medicare MSA plans are not a fit for everyone. The following conditions disqualify enrollment per the Fenyx Health 2026 Summary of Benefits:
❌ Other medical coverage that covers the MSA deductible (employer or union group health plans)
❌ TRICARE coverage
❌ Veterans Affairs (VA) benefits
❌ Federal Employee Health Benefit Plan (FEHBP)
❌ Medicaid eligibility (dual-eligible)
❌ Currently receiving Medicare hospice benefits
❌ Residing outside the U.S. more than 183 days per year
